Join at your visit, and that visit is included
New or existing patient, when you sign up at your appointment, that day's exam, cleaning and x-rays are covered by your membership (new patient visits are a value of up to $340). You pay the one-time $149 activation fee plus your first month ($188 Adult or $212 Perio), then the monthly fee after that. Treatment done that day is billed at the member rate with 15% off.

No. The membership is for patients without dental insurance. It is not insurance and cannot be combined with insurance benefits for the same services.
Adult Membership is $39 a month and Perio Membership is $63 a month. There is a one-time $149 activation fee. When your membership starts, you pay the $149 activation fee plus your first month: $188 for Adult or $212 for Perio.
Adult: 2 professional cleanings a year, 1 to 2 doctor exams, all clinically necessary x-rays, 1 emergency exam a year, and 15% off eligible treatment. Perio: 3 to 4 periodontal maintenance cleanings a year plus the same exams, x-rays, emergency exam and 15% discount.

Yes, you can cancel anytime. The activation fee is non-refundable. If your included visit was a new patient visit and you cancel within 6 months of joining, Elite Dental may charge the remaining balance of that visit fee.
